Step One:

Our Geriatric Care Managers conduct a Comprehensive Geriatric Assessment to identify the senior’s physical, mental and social abilities, including ability to perform daily living activities such as dressing, bathing, meal preparation, medication management and more. When placement is an immediate goal- there is no charge to you.  If we provide Geriatric Care Planning Services- we have a fee schedule that is reasonable and affordable.

Step Two:

A comprehensive Care Plan is compiled and documented for the client, family, and if needed, your physician that identifies problems and includes a step-by-step process to achieve identified goals and solutions.

Step Three:

Our Geriatric Care Managers use a team approach to help your loved one by working with our large network of Community Partners who are committed to the mission of ethically working with seniors and their families. We put the pieces together for each individual, saving you time, money and emotional stress. We are your advocate!
